A woman goes on vacation with her family and friends and her husband and her daughter encounters a tragic accident. One year later she goes hiking with her friends and they get trapped in the cave. With a lack of supply, they struggle to survive and they meet strange blood thirsty creatures.

Sarah manages to escape the caves covered in blood and it not hers the sheriff and his deputy force her to go back to the cave because she cant remember what happened to her and her Friends she soon will wheen the horror all begins and the body count starts to rise the fight

The sequel will continue the story of Sarah, who as the only survivor of an all female caving expedition gone horrifically wrong, suffers severe psychological trauma. Unable to speak, she accompanies a rescue team back to the cave, but events once again take a sinister turn.

An experienced rescue team composed by Dan, Greg and Cath is trying to locate the six women in the caves of the Appalachian Mountains. Meanwhile Sarah is found wounded, covered with blood, in shock and amnesic by a truck driver and brought to the Hyett County Hospital. Sheriff Vaines suspects that Sarah has killed her friends and forces her to return to the Boreham Caves together with his Deputy Rios and the rescue team. They search deeper and deeper in the caves but when Sarah recovers her memory, she panics and Sheriff Vaines shoots, provoking a cavein. The trapped group sooner faces the hunger and savage breed of predators while trying to find a way out.
